If you wish to register yourself on the "Hong Kong SMA Member Database" and want to be kept informed of the developments, please choose either version, download and fill out the following Member Registration Form. Completed forms could be submitted to our organization by email or fax. The Trust would examine and approve the application for one to become an official member.

SMA patients and their family members are eligible to apply for help from Families of SMA. Applicants can click the boxes to download, print, complete and send to us by fax at 2510 7199 or by post to Families of SMA Charitable Trust, Room 1402, 14/F, SUP Tower, 83 King's Road, North Point, Hong Kong.
